Please help! I want to know about the super پرستار medal.

I have been gone from fanpop for a few years and today when I came back I see a مالٹا, نارنگی medal that says super پرستار so I wondered are there 4 تمغے now یا did super پرستار replace a medal? What level is super پرستار and why did they add a 4th medal? I thought 3 was good.
