The system isn't entirely clear - the F4 want to keep some mystery in it, probably to stop people from cheating in medal-gaining. However, we know it's mainly from being active in کلب and adding good contributions to them. The مزید people پرستار your images, links, مضامین and videos, the مزید likely آپ are to get a medal there. Answering پولز and سوالات and commenting on things also helps. It is a bit random though. And I think there have been some glitches/weird occurrences of getting تمغے in کلب people haven't been that active in.
